How to Fill Out an Assisted Living Facility License Application

Filling out the ALF license application might seem confusing. There are so many forms, fees, and details—where do you start?

Don’t worry. This guide walks you through it step-by-step so you can feel confident and avoid costly mistakes.

Why Your Application Matters

Your license is the key that unlocks your entire business. Without it, you can’t admit residents, open your doors, or fully market your home (though you should still be letting people know you're getting started!).

Before you submit, make sure you:

βœ… Choose the right license type
βœ… Calculate your fees correctly
βœ… Submit all required documents


Step 1: Fill Out Your Basic Facility Information

Start with the basics:

πŸ“Œ EIN (Tax ID number)
πŸ“Œ Type of license you’re applying for (Standard, LNS, ECC, LMH, etc.)

πŸ’‘ Pro Tip: If you’re unsure about the license type, call the state. They’re there to help and will guide you based on your business model.


Step 2: Calculate and Include the Correct Fees

Here’s where people get tripped up. The fees are per bed, and they vary depending on license type.

Example for a Standard ALF License:

βœ… $64.96 per bed
βœ… $387.73 application fee
βœ… $2 per bed biennial fee

Step 3: Add Ownership and Management Info

This section includes:

Step 4: List Your Key Personnel

The application asks for:

🎯 If you're wearing all the hats (owner, admin, etc.), that’s perfectly okay—just list your name for each.


Step 5: Answer All Disclosure Questions Honestly

for that um let's see um let's get into the topic for today filling out this facility application to to obtain the license for the facility so as you're going through this um a couple things to know the this is different from your administrator license um so I talked to you a little bit about that here at the beginning there's two different licenses that you have to have um for the facility in order to run it there's the administrator license this does not have to be you as I mentioned I work out of

state so I don't I don't have an administrator license and that's okay being the owner of the business it's fine I do have to have somebody that has a facility administrator license at my facility that is a requirement it does not have to be need them that is not what we're talking about today though if you would like um a deeper dive on that type of application process what it takes to become an administrator type in administrator down below and if I get a bunch of people commenting that then

that'll be a cue for me to uh create a video on what it takes to become an administrator of an assisted living facility also of note it each of these applications are going to be different for every single state um they are not it's not a standard application so what that means is we're going to go through an example here and I believe it's in Florida um you could be in California or Texas or Delaware or South Dakota or Washington I wherever you're at the application itself is going to look

slightly different because it is a state specific application what we're going to go through should be applicable to your state though because while the application looks different it's going to have a little bit different wording might need a few different things um at its core it's going to be a lot of the same information so um just want to make sure that you're aware of that now the other thing that you want to be aware of as you're getting into these applications is there are fees that are

associated with them and we'll we'll see this when we get into the application the example here there are two fees though that you need to be aware of there is a General application fee so that is they'll say you know in order for you to to get the license to operate this it's $10,000 or $5,000 so there's there's a chunk of money that you have to pay for the license for the facility and then there will also be a bed count fee and this bed count fee is how many beds you want to be licensed

for and the type of um care that you're going to be providing so this could be anywhere from $50 per bed to $500 per bed it just kind of depends on the state and how they want to do that so and you'll see this again when we do the application example you let's say it's $5,000 for the Bas And1 $ for every single bed after that you would pay 5,000 Plus in a 10 bed facility 10 beds times $100 is ,000 I can't do the math doing math when it's live is is just a disaster so you

know I think it's a thousand um so you'd pay that 5,000 base and then You' do the Thousand um for the bed count so you'd be paying a total of $6,000 for um for what you need to pay so that's what the fees are going to look like and how they work as you get into that application it can be confusing to kind of navigate that and understand what it's saying we'll get into this in the example but just understand that there are fees associated with this it's not a free

application so you do have to put a little skin in the game in order to to get this up and running it's not a huge amount but it is a chunk of money that you're going to have to have and set aside to to get this going there's also going to be other items that you need as you are going through the application process um there's the the actual physical application that that we'll look at here in a few minutes um itself they're going to want bank account information they're

going to want to see insurance information like a Certificate of Insurance they will want entity documents so this could be your operating agreement and or your EIN letter that says I have an LLC or I like I'm really a business I promise here's the paper that says that I'm a business um so you need to have that you're going to have to have your policies and procedures um this is a big thing and this is what can take a really long time as you're getting your facility license application put

thank you for all the work you do there um there may be other requirements as well um that are just State based and and things that you need to be aware of as you're as you're getting that the whole application they will tell you what it is um it's probably not going to be on the physical application itself but they will have a list a punch list of we need these things here uh typically it's going to be these things on here your insurance and maybe one or two other things that you'll need it's

days or weeks to fill out this application so uh okay awesome thank you jacine uh okay I uh okay right here at the top of this this is your provider information and what you're going to be putting in here is the information as a provider some of the stuff you may not end up having um you know you might not have a license number you may not have an NPI I would check with your state to see if you need an NPI an NPI is the national provider identifier you can register for it for free um but Most states do not require

it um if if you end up running into that problem please reach out to me and I can kind of give you some guidance on that if I see enough people will do a kind of a video on how to obtain an NPI Most states for assisted living do not need an an N an an NPI uh it's often used for billing and you don't have to worry about that um same with Medicare so in in Assisted Living we don't deal with Medicare and so you do not need to have the CMS CCN um you do not need that that number there um if you are a Medicaid if

you have Medicaid then you could put that in there most of the time when you're going through this you will not be able to become Medicaid certified until you have your facility license so you're probably not going to need that either okay so that's that whole Top Line you probably actually don't need a lot of that information you can check that they looks like they have resource links and things like that go ahead and check it um to see but probably not going to need it um now this is just

more basic information name of the facility addresses websites email addresses contact info all of that stuff so you're going to put that on there the property owner information so if it is um if it's I think if it's the same as the information here then you don't need to do it if it's different then you're going to check that and you're going to just put the the information on here what that means is you have two entities let's say one entity owns the real estate and one entity is the operating

entity you would have a separate company that owns the property you would fill this in here if it is the same one that owns it then you would say no um so that's how you would treat that one um let's get into contact person typically this is the person filling out this information in case they need to reach out how are they going to contact you here's the license information right here so this is the the lcy name this is the name of the owner of the facility your EIN number um for your

LLC your contact information here and then the description of the leny what type of entity are you are you a for-profit not for-profit are you a public like a state um a government owned entity um you're probably going to be for profit or not for profit and then you need to just check check the box that you are I would you know in this example I would probably put it as an LLC a limited liability company but you would put whatever it is that you need to put on yours this is where you're going to get

into the application type and this is where there's a lot of confusion so you would say if you're doing an initial license this is what you're going to do if you're doing a change of ownership you're going to do this right here or if you're doing a renewal you're going to have to fill this out this renewal is where at the top where it was asking for all that information you have that at that point and you can put it in there um also a lot of States when you do a renewal they have an online portal for

it so you don't have to fill out this paper application every single time which is nice um but if you needed to drop it to paper this is how you do it and you go through this process right here so what you're going to put in here um this is where I say to people initial license or change of ownership very similar to same application as you can see here you check this box or you check this box and you just kind of put the things in there that that make that are applicable to you and what you're trying

to do um now if you are doing fee if the fee is required um you're going to have to put some of these things in there um there should be some guidance here um let's see Personnel so this is um these are the things that if you're doing this on the application there's a fee that is associated with it if you're making a change to something like this then there is no fee required it's more of a FYI we're changing this so if you're doing a new provider name new provider address

um bed capacity we're changing that uh any of the services that we're providing are changing there's a fee associated with that you have to put that over here if you're just saying we had a new partner come on um the property owner has changed they need to know that information you're going to fill out the the application here but that is a no fee required um type of a of a situation okay um these are also um pieces that go up there didn't the PDF doesn't look like it came through um great but you

know there's government for you um so this is the the type of license um what type of what type of services are you going to be doing um are you Nur limited nursing limited health um Extended congregate care um there are there's definitions for these that are easy to find online I'm not going to dive into every single one of them but what type of license are you going to be doing how many beds how many buildings how many Wings floors rooms um you want to put all of that if you're extended

congregate care um you need to put all of that in here um for an LNS these are specific questions for the LNS and the ECC license here's the lmh license and again limited mental health L lmh LNS is limited nursing services and ECC is extended congregate care when you're getting into assisted living I believe you're going to be ECC that's probably the license but it depends like are you going to be doing Mental Health stuff and you need to have an lmh license those are questions you need to dig in

for your specific situation and asking somebody at the state for some extra guidance is going to help you out quite a bit in figuring that out here is where you're going to be doing the number of beds so you're going to say you know do we do we need extra beds here how many beds are we doing this um weit the fee for the OSS beds and right here this is what we're talking about a per bed fee of 6496 in this case um for the osss which is I don't know if I have the acronym on here I'm not seeing it

um yeah not sure what OSS is but you could probably find that pretty quickly uh and figure out what that is but you would say um we are going to have um 10 beds at 6496 that's $650 uh you know $649 60 that's how much you're going to have in that fee for this specific thing around the bed count um how how you do that math it's very very simple right there okay um this is where OSS beds I'm wondering if it's something like it's a kind of like a Medicaid type of a thing but I'm

not 100% sure uh for those of you that are working in Florida it's an acronym that I don't know you can very quickly figure out what that is okay this is the lure fee down here that you need to to be aware of um so this is the fee that we're talking about up front so so there's going to be this is just basically a little grid that shows you what this is going to look like so this is let's say that we're doing a standard Alf um license fee except exemption um you know do you have

the exemption or not and you probably don't if you're doing this for the first time um what you're going to do here is let's say that this this this initial one here 649 6496 per bed times 10 so that's $649.90 plus this 38 8773 um and that's that's your total fee that you need to be aware of now if you're doing an ECC license then you're doing 1015 per bed and then adding the 546 if you're doing LNS 1015 plus 322 77 lmh there's no extra fee um banial assessment fee so that's I'm guessing

that's like in every other year you have to do just kind of an assessment thing where it's um $2 per bed um and you just have to do that okay so this is how you can figure out the fees each of these application period each of these application forms will walk you through how you're going to to do this this is if you're going to increase or decrease the bed capacity um between license renewal periods um so you can figure out that information here as well to put in there this is a change during the

licensing period so it's a fee so if you were increasing or decreasing during like an initial um while you have an an actual license you have to notify them that we're doing this um adjust your fees you have to do like a $25 processing fee if you are ending up doing this and during a renewal period between those periods um I would suggest reaching out to the state for some guidance on exactly what that looks like and what the fees associated with it are you can see here in Florida that these

are not a ton um 1015 per bed and 6496 per bed those are very reasonable um compared to what I've seen in some other states so uh it's something for you to just kind of look at and you're just kind of doing the math on here um this is specialty between license so if you're adding some kind of a specialty license on these as well you're going to have to pay extra money um so that's just how that works helps you figure out what the fees are because these acronyms are specific

to Florida it's hard to say exactly which one you would be doing and so what I would say is reach out to the state um get clear on which one you need to do in your specific situation and then use this to calculate what those fees are going to look like for you okay um down here this is ownership information um who the owners are how much of the company The Entity they own um you're going to put that in here board members um so you have to fill this out this is this is pretty standard um as well I see

this on on multiple I've seen this in multiple States um on their applications if you have a management company then um you're going to put this in here and who the management company is if if you don't then you just hit no and you can skip it because they don't you don't have a management company if you do then you're going to put that if you have a management company you're going to know that because you're going to hire them typically a management company you're

going to pay about 5% of gross income too um and smaller facilities most management companies don't want to work with small facilities it's like a single 16 bed or smaller um they're not that interested because there's not a lot of um there's not a lot of income that they get off of that and so they're probably going to be less likely to do it just if you want to know more about management companies type in management company down below and uh we can do a video on that at some point in the future this is

the Personnel um that are involved in the facility so this is where your facility administrator you're going to need that on here this is where if you are the administrator you need your license number so you can put that information on here um when the training happened if you're not the administrator you need this information from your administrator so you can put that on there um who's managing it all their information on there this is the person that's in charge of finances and this is

just contact information that they need there and then the safety person oftentimes in a small facility this will be the facility administrator could be you as the owner um but they need to know who the safety person is in case they need to to get in there uh this is these are just required disclosures I'm not going to read them um you can look at them and say what what we're are going to disclose or not disclos CL um and you have to uh indicate um what you're seeing on there a lot of these

are questions about you um you know are you have you ever committed fraud is there a history there it's kind of background information um that you fill out yes or no just be honest uh as you're filling that out um these are provider fines and financial um information so if there are fines or anything anything like that that you need to be aware of then you'll do this Most states will have an online portal that uh give you this information um here as well so you're not probably not

going to need to fill that out but um if you ever need to drop it to paper I would ask for guidance from the state um to to get some specific guidance there consumer information this is um a little bit of information about the let's see currently apply to your facility so this is um asking it's it's kind of like a survey about your facility and often times this is going to be with a relicense because on a brand new facility you don't have any of this information but do you have um what's your license

capacity how many private rooms and semi-private rooms are offered uh do you have a bed hold policy that you hold things for people any religious affiliations any languages that are spoken in the facility a lot of this is like going to be public information that goes out and they put it on their websites so that people can um you know receive guidance or or direction as as to what they need um there uh this is availability of nurses that are on site um do you have nurses are they direct staff are they third party um are they

24-hour are they just part-time what does that look like so you're going to put that information on there uh defines what nurse is LPN R RNs um APRN what does that look like um you just want to be honest there um what payment forms do you do um Medicare Medicaid veterans uh Insurance you know all of those things what do you accept um you can put that in there recreational programs do you offer anything extra for your residents um that you want to put in there special care units and programs you know do you

offer anything extra like that on here here's a supporting documents so this is what we were talking about as far as the other documents that you might need these are requirements for the State of Florida so it's going to get in here all the other things that you need to have so I said five to seven obviously this is a lot more so you just want to look at that and if you have questions on what some of these things are like assurity or Continuum Bond I don't know what that is and so I would reach out to

the state and say or do an internet search and and try to figure out exactly what that is and and what it means for me um but you you got to fill out those this proof of financial ability to operate it's telling you go find this form so you would just go in and find that form online and fill it out and attach it um and and you're just going to put all of that U put put it all together policies and procedures um all the all this stuff ATT testation this is just saying I is the the owner of the

facility um I attest to all of these things um you sign it and you know date it and everything there and that is how you put this together um it it really is there there's not a lot here the most confusing pieces of it can be trying to figure out what the fees look like and and how to understand what that is um and then maybe which population you want to serve so we're talking about the ECC license versus the versus the lmh license and and what that looks like those are going to be specific to States they specific
